As part of our case management team, Discharge Coordinator Jessica Winters supports patients and their families as they transition from hospital to home.
From medical equipment to medication, she works to ensure patients have everything they need to continue their recovery.
“What can we do to keep you safe at home? What can we do to lessen your stress while you’re healing? We try to make it as easy on patients as possible when they discharge.”
Patient needs vary greatly. Some patients require home healthcare or physical therapy, while others need assistance coordinating meals and transportation. Whatever the case, Jessica and her team are ready to assist.
“We are a resource, whether there’s a healthcare need, addiction need, swing bed need. We’re always happy to answer questions and get you to where you need to go.”
