If you’re 65 or over, on Medicare, and have two or more chronic conditions, you may soon receive a call from our care team. Managing chronic conditions can be overwhelming, but you’re not alone on your journey. The ChartSpan care management program provides a host of benefits, including:

- Appointment scheduling
- Assistance with prescription refills
- Personalized healthcare plan
Chronic care management programs have been proven to reduce hospitalization by helping patients be proactive with their health.
Although our representatives can help you make an Annual Wellness Visit (AWV) appointment or provide information on the care management program, know that they will never ask for personal information like your credit card or Social Security number.
